You have the right to be treated with respect and dignity and to have your property treated with respect.
You have the right to choose whether or not you wish to receive the services of our in-home aide program.
You have the right to be fully informed in advance about the care and services to be provided by our department and the limits to those services.
You have the right to be informed in advance of any changes to be made in your plan of care or services to be provided.
You have the right to participate in the development of your plan of care and in any modifications of the plan.
You have the right to be informed in advance about any fees or cost-sharing policies that may apply to the service you are to receive.
You have the right to voice grievances without fear of reprisals or discrimination or loss of services.
You have the right to privacy and confidentiality of personal information.
You have the right to be kept fully informed of your rights and responsibilities.
